The Medical Record Department of Wiregrass Medical Center is open and staffed Monday through Friday from 8:00 A.M. until 4:30 P.M.
The Medical Record Department coordinates all activities relative to the documentation, usage, retrieval and storage of all medical records. We protect your health care information in compliance with federal and state requirements. A patient has the right to request a copy of his or her medical records. However, a completed written authorization is required prior to processing a request. Please see the link below for the written medical record authorization. Health care providers can freely share information for treatment purposes without a signed patient authorization.
What you need in order to get a copy of your medical records:
You will need a completed written authorization.When you come to pick up your medical records, please bring a photo ID.
There may be a charge for creating a copy of your records. If copies of the record are going to another physician or another facility there will not be a charge.
If someone has Power of Attorney, the paperwork must state that medical records are included.
If a patient is deceased, we need a copy of the death certificate. We can only release records to the living spouse or Executor of the Estate.
Any patient 14 and older must sign his or her own medical release. Records of patients under the age of 14 can be released only to their legal guardian.

Coroners, Medical Examiners, Funeral Directors: Wiregrass Medical Center may release your medical information to a coroner or a medical examiner. This may be necessary, for example to identify a deceased person or determine a cause of death. Wiregrass Medical Center may also release your medical information to funeral directors as necessary to carry out their duties.
